First Visit
Your initial appointment will consist of a consultation which will allow Dr. Dyriw to explain your diagnosis and treatment options. Depending on the situation, we are occasionally able to provide treatment the same day. A complex medical history or treatment plan may require a second visit to begin treatment on another day.
Please be sure to bring the following items with you to your appointment:
- Your referral slip and if possible, any x-rays from your dentist’s office.
- A list of medications you are currently taking.
- Any necessary dental and medical insurance information if you have insurance.
- Driver’s License or Photo ID.
IMPORTANT: We require the accompaniment of a parent or guardian for all patients under the age of 18.
Please continue taking any medications (diabetes, blood pressure, etc) as you regularly would unless instructed otherwise. If you have any questions regarding this, please contact our office.
Please eat a full breakfast or lunch as applicable.
All procedures will be performed using local anesthesia. There are usually no restrictions after the procedure concerning driving or returning to work.
If your physician has advised you to use an antibiotic premedication because of a heart condition or joint replacement, please make sure you take this prior to treatment. If you have any questions, please contact our office.
